| There are 3 types of application forms for 6 Categories.
The categories are as follows: |

Application form for retired Army Officers – Category “B”
|

| 2. |
Application form for Serving Civilian Officers
paid out by Defence Authorities – Category “H”

|
| 3. |
Application form for allotment of plot for civilians as defined
below: |

HQ Hospitals or nearest CMH to prove their disability. |

All Federal and Provincial Govt./Semi Govt.
Officers of Grade-17 and above are eligible to apply on this quota
subject to the Certification from the Head of the Dept. |

All Citizens of Pakistan more
than 18 years on the last date of submission of application i.e.
28th August, 2009. |

Pease note that the ‘On Duty
Army Officers’ can apply through GHQ; their end to end process will
be directly handled by GHQ from obtaining the application till final submission
at DHA Khi Office. Additionally if any Army Officer wishes to apply, they
can approach any of MCB's designated branches and use the application
form of type 3 category. |

168 designated MCB Branches will
accept applications from 28 July 2009 till 28 August 2009. (Branch
list attached for your convenience) |

| 2. |
The payments will only be collected via pay
order / demand draft. (Payment in cash will not be acceptable at
any MCB branches). |

|
| 3. |
Overseas Pakistanis will be
forwarding the application forms along with required documents directly
to DHA Karachi office address mentioned on top of the application
form, applications from Overseas Pakistanis will not be processed
through MCB branches. |

| 4. |
Two latest passport size photographs and two
attested photocopies of CNIC of the applicant are required along
with the application form. A copy of CNIC of spouse (Next of Kin
incase of other than spouse) is also required. |
| |
|
| 5. |
Down payment of un-successful applicants will
be refunded by 26th Nov ’09, through the same branch where
the applicant deposited his/her application (Refund of unsuccessful
applications will only be through pay orders). |
